Edgbarrow Sports Centre holiday fun club booking form

 

About this form

Use this form to book children's holiday activities at Edgbarrow Sports Centre taking place 25 to 29 October 2010.
This form can only be completed by the parents/legal guardians of the child/children you are booking.
You can book up to four children on this form provided all children have the same emergency contact details.

What you will need

You will be asked to enter:

  • Your contact details
  • Contact information of two people in case of emergency
  • Personal details for yourself and your child/children
  • Any special or medical needs for your child/children
  • The activity session(s) you wish your child/children to attend
 

What happens next?

We will contact you within 48 hours of receiving your form to discuss your booking.

Please note that your booking is not confirmed until we contact you to confirm availability and process payment.
